DATA PRIVACY NOTICE & CONSENT FORM

Purpose of Data Collection: In accordance with the Data Privacy Act of 2012 (RA 10173), the SCUAA 1 REGIONAL GAMES 2026 Organizing Committee will collect and process your personal data for organizing, monitoring, documenting sports meet activities, and supporting university accreditation requirements.

Personal Data to be Collected: Full Name, Birthdate/Age, Contact Number, Email, Course/Year, and Photograph.

Use of Personal Data:

• Registration and participation in the sports meet.
• Communication regarding schedules and results.
• Publication in the online sports meet gallery and event documentation.

Data Retention & Protection: Your data will be stored securely and accessed only by authorized personnel. It will be permanently deleted once its purpose is fulfilled.

Rights: You have the right to access, correct, or request deletion of your data via the SCUAA-1 President.
